Medical and Dental Benefits
The Company offers a variety of medical/dental plans and options. Choices include an in-network and Point-of-Service Plan. Both plans provide 100% coverage for all age based preventive procedures. Employees in California, Florida, Georgia, New Jersey, Oregon, Missouri, Pennsylvania, and Texas are offered a dental HMO, as well as a dental PPO.

Health Incentive Program
Employees have an opportunity to reduce medical plan premiums while at the same time receiving important health information to share with their physicians.

Vision Plan
Employees can enroll in a nationwide eye care program that offers substantial savings on all eye care and eyewear needs.

Short-Term Disability
The Company provides a short-term disability benefit that bridges 40% of weekly earnings during a qualified disability. Employees can enhance this benefit with two reasonably priced buy-up options.

Long-Term Disability
Through this program, employees are eligible to receive income protection in the event of illness or non-work related injury that prohibits them from working for six months or longer. This benefit is fully paid for by the Company.

Group Life Insurance
Our employees take comfort in knowing that their families are protected thanks to our group life insurance program. The Company provides a base level of $50,000 term life insurance and numerous buy-up options are available for employees and their spouse/dependents. Optional Accidental Death & Dismemberment insurance policies are also available.

Medical Plan Opt-Out Credit
Employees may elect to secure medical coverage through their spouses plan or an alternative plan. The Company will provide these employees with $500 if they do not take the Company's medical plan for an entire year.
